Personal Medical Record software for your information. If

Will My Life Record Work On My iPod touch?
Yes, the My Life Record software will
function on both the iPhone and the iPod touch. The same
technologies are used in both devices, and My Life Record
utilizes those technologies. The iPhone always has an
available network connection via the cell phone carrier,
however the iPod touch does not. In order to use My Life
Record with an iPod touch, you will need to have an
available Wifi connection.
How Do I Get My Health Records Into My Life Record?
My Life Record has an easy to use function
that allows you to get your medical records from your
physician and into your My Life Record profile with ease.
You simply touch an icon on your iPhone and your iPhone
will provide you with a fax number and a code. Simply
provide this fax number and code to your doctor. All your
doctor has to do is print or write that code on the front
of the fax and fax your chart. My Life Record will take
your chart and tie it to your My Life Record profile,
creating an exact copy of your medical chart in your My
Life Record profile. Your doctor does nothing more than fax
your chart, just as he or she would do if he or she were
giving your record to another physician for treatment.
What If My Doctor Makes A Mistake In My Chart, Can
He Change It Later?
Absolutely not. Any entries made into My
Life Record are permanent. If your physician (or anyone for
that matter) puts something into your official My Life
Record medical chart it's there forever. This is for your
protection, because you are guaranteed that what is in your
chart stays in your chart without fear of tampering.
Can My Doctor Add Anything He/She Wants At Any
Time?
No. You must authorize your doctor to add
things into your chart. You can authorize your physician
access for a day, a year, 3 years or anything in between.
Your record is only accessible if you specifically allow
it.
If I Share My Record, Can The Person I Share It
With Change Anything?
No. Sharing your record allows whoever you
chose to share it with view access, but they cannot change
anything. Again, everything that goes into My Life Record
is permanent, and cannot be changed.
If I lose My iPhone, Can The Person Who Finds It
See All Of My Information?
In the event you lose your iPhone or iPod
touch, contact My Life Record support immediately. We can
disable the device so that your health information is no
longer accessible via that device. Should you find your
device later, simply let us know and we can re-enable it as
well. As long as you notify us that your iPhone or iPod
touch is lost or stolen, we can prevent it from accessing
any of your healthcare information.
If I Get A New iPhone or iPod touch, Do I Have To
Re-Enter My Information On It?
No. My Life Record allows you to sync your
device with your My Life Record profile, thereby granting
it access to all of your healthcare information that you
already have on file by simply entering a device
synchronization code.
If I Have Multiple iPhones or iPod touches, Do I
Have To Manually Enter My Health Information On Each One Of
Them?
No. My Life Record will keep all of the
devices associated with your My Life Record profile synced
automatically. If you add a note on one iPhone and view the
notes on another iPhone it will be updated instantly to
reflect the notes you just entered, on all of your devices.
This is the same for all aspects of your health record
including imaging, medications, charts, etc.
When My Life Record receives faxes from my doctors,
what happens to the fax when My Life Record is done?
When a physician sends a fax to My Life
Record, it is never put to paper. It is instantly digitized
and placed in your My Life Record medical profile. When a
fax comes in, it is not printed, therefore is not discarded
in a trash bin. It is entirely electronic for your
safety.
