My Life Record Privacy Policy

1. PRIVACY AGREEMENT
WITH LIFE RECORD INC.
Your use of My Life Record is governed by this privacy
agreement and does not supersede the End User License
Agreement (EULA). "My Life Record" and “Life Record Inc.”
means Life Record Inc., located in Bloomfield, New Mexico,
United States, and its subsidiaries or affiliates involved
in providing My Life Record as well as the software and/or
services that make up the My Life Record application.
2. OUR PHILOSOPHY.
Your medical record and its integrity is our greatest
concern. You will find once something is entered into My
Life Record that is a medical item you cannot change it in
My Life Record. Many doctors, hospitals, insurance
companies and health record services allow records to be
modified and/or deleted. My Life Record makes entries
permanent. We also do not provide external companies any
information to your health profile. An insurance company,
physician, hospital, etc will never have the ability to
make changes to your my Life Record medical record, ever.
You can be assured when your medical record is attached to
your My Life Record Profile; it will stay exactly as
submitted without change by anyone. If an entry requires
correction, the entity (doctor, hospital, etc) can append a
document stating the change, but cannot remove any
information previously put into your health record.
3. CHANGES TO POLICY.
While our philosophy will not change, the details of this
policy will change over time as new technologies are
developed. You can view any updates, as well as the date
they take place by visiting the My Life Record website.

5. SCOPE OF THIS PRIVACY POLICY.
This Policy applies to the usage of the My Life Record
application and/or service, and the information contained
therein.
6. SHARING AND USE OF INFORMATION.
We do not share or sell your information to anyone for any
reason. The only time your information is shared with
anyone is when you specifically authorize it through your
My Life Record profile or when permitted by you for support
purposes.
7. PHYSICAL SECURITY AND PRIVACY.
The facilities that house our servers, network devices,
backup storage media and other equipment and information
are physically secured and attended. Access is strictly
limited to only those individuals who require it for
legitimate, documented business purposes. Those individuals
will have access to the minimum amount of information
necessary to accomplish their business related task.
Life Record Inc. takes all reasonable steps to provide
secure transmission of your information from your computers
/ mobile device to our servers. When you transmit personal
information to us, we encrypt it using Secure Socket Layer
(SSL), the industry-standard encryption technology.
Life Record Inc. employees and contractors have occasional,
legitimate needs to access our data servers for purposes of
system troubleshooting, maintenance and end user support.
Such access is permitted only for those who have such needs
and are only permitted to access the minimum necessary
amount of information. All such individuals have signed
confidentiality agreements with Life Record Inc. Access is
controlled via pre-assigned user accounts. Our systems are
monitored to detect breaches in security. The My Life
Record application is designed to meet or exceed regulatory
and industry self-regulatory guidelines regarding privacy,
confidentiality, and information security. It is our policy
to review and adapt to statutes, regulations, formal
private-sector standards, and informal policy guidelines as
they apply.
8. DATA MINING AND USE.
Life Record Inc. does not data mine your health
information. We do not use your health information in any
manner, except when requested by you for support purposes.
Life Record Inc. collects and uses network utilization and
activity statistics so we can understand the ways in which
our sites and services are being utilized. This data is
stored anonymously and it is only used in the form of
aggregated statistics.
9. LEGAL REQUESTS.
Life Record Inc. cooperates with law enforcement inquiries
as a matter of policy. We use IP addresses and other
available information to attempt to identify a user when we
are legally compelled to do so or when we feel it is
necessary to protect our services, sites, customers, or
others. Law enforcement inquiries are done only through
court order or subpoena requests.
10. LEGAL DISCLAIMER.
Unfortunately, no data protection method or combination of
methods can be guaranteed 100 percent secure. We take all
reasonable steps to protect your personal information as
described throughout this policy, but we cannot ensure or
warrant our ability to do so. As a result, you use My Life
Record content, products, and services at your own risk.
Life Record Inc. will not be liable for disclosures of your
personal information due to errors in transmission or
unauthorized acts of third parties.
